ADOPTION AGREEMENT

Having received the German Shorthaired Pointer _______________, AKC # (if Available) ___________ through the German Shorthair Pointer Care of Ohio Rescue, Inc. (“Rescue”), I/we willingly agree to the following terms and conditions of adoption:

1. Adoption Donation. The adoption donation for this dog is $________. No refunds will be given.

2. Returns. If for any reason this adoption is not satisfactory to the adopting party, or if the adopter(s) fails to comply with the terms of adoption at any time during the life of the dog, the dog must be returned to Rescue. It may not be given or sold to anyone else without the written permission of the rescue agent involved, nor may it be released to a shelter or pound.

3. Training. The dog may not be protection or attack trained or agitated in any manner or used as a guard dog for any agency, firm, individual or corporation. The dog will not be maintained as an outdoor yard dog in any fashion, nor will it be used for any experimental or fighting purpose. The sole function of this dog and any rescued GSP is as an in-home companion animal and potential personal hunting dog, although Rescue makes no representations or warranties regarding the hunting abilities of any rescue dog, including this one.

4. Health and Welfare. This dog’s optimum health must be maintained through the feeding of quality dog food; adequate shelter and exercise; annual physicals and inoculation with the adopter’s veterinarian; and heartworm and other parasite testing and prevention.

5. Identification. This dog must wear identification at all times. It must be licensed and have personal identification. Rescue recommends tattooing or microchipping in addition to collar tags.

6. Loss. If the dog is lost or stolen, Rescue must be identified immediately.

7. Reservation of Rights. Rescue reserves the right to arrange a follow-up visit or other contacts to ascertain if all conditions of the adoption are being upheld. Should the adopter fail to uphold any term or condition of this agreement, Rescue reserves the right to terminate this agreement and have the dog returned to the Rescue program.

8. Attorney’s Fees and Other Costs. Should it be necessary for Rescue to take legal action to recover this adopted dog or otherwise enforce provisions of this agreement, the undersigned adopter(s) agree to pay all court costs and attorney’s fees.

9. Release. The undersigned agrees to release Rescue and its agents and volunteers and their heirs from any and all responsibility for any claims, actions, losses, damage, penalties, fines or costs adopter may incur as a result of this dog’s actions. Adopter understands the risks and responsibilities involved in dog ownership and willingly agrees to accept all such risks and responsibilities associated with the adoption of this dog. The undersigned has read and understands all terms of this adoption and agrees to abide by the terms and conditions of this agreement.
